Caloric labels do not influence taste pleasantness and neural responses to erythritol and sucrose

INTRODUCTION: The beneficial effects of substituting sugar with non-caloric sweeteners (NCSs) remain uncertain due to the mismatch between their rewarding sweet taste and lack of energy content. Functional magnetic resonance imaging (fMRI) studies indicate an influence of cognitive processes (e.g., beliefs, expectations) on reward system responses to NCSs, thereby changing their rewarding properties. We measured the impact of cognitive influences about the caloric content on brain responses and liking ratings to erythritol, a natural NCS with satiating properties, versus sugar (i.e., sucrose). METHODS: We performed a within-subject, single-blind, counterbalanced fMRI study in 30 healthy males (mean ± SD: age 23 ± 0.6 years, BMI 22.5 ± 0.3 kg/m²). Concentrations of erythritol were individually titrated to match the perceived sweetness intensity of a 16 % sucrose solution. During the scan, sucrose and equisweet erythritol solutions were delivered as 1 mL sips with either correct or purposefully incorrect "low-calorie" or "high-calorie" labels. After each sip, participants rated sweetness liking. Water with a "water" label was used as the control condition. RESULTS: A 2 × 2 ANOVA revealed lower liking ratings for erythritol than sucrose (p < 0.0001), but no main effect of the label, nor label-by-sweetener interaction. General Linear Model (GLM) analysis of brain responses at FDR q < 0.05 showed no main effect of sweetener nor label, nor a label-by-sweetener interaction. However, several patterns of brain activity mediated the differences in subjective liking ratings between the sweeteners. Moreover, different neural responses were found for sucrose vs. water in parcel-wise, SVM, and ROI-based analyses, whereas for erythritol vs. water, only the latter two showed differences. Lastly, sucrose induced a stronger craving signature response compared to erythritol, driven by the pattern specific to drug craving. CONCLUSION: Liking ratings were lower for erythritol than sucrose, and they were unaffected by the caloric label. There were no differences in neural responses between the sweeteners and labels, except in comparisons with water.

Cross-paradigm integration shows a common neural basis for aversive and appetitive conditioning

Sharing imaging data and comparing them across different psychological tasks is becoming increasingly possible as the open science movement advances. Such cross-paradigm integration has the potential to identify commonalities in findings that neighboring areas of study thought to be paradigm-specific. However, even the integration of research from closely related paradigms, such as aversive and appetitive classical conditioning is rare - even though qualitative comparisons already hint at how similar the 'fear network' and 'reward network' may be. We aimed to validate these theories by taking a multivariate approach to assess commonalities across paradigms empirically. Specifically, we quantified the similarity of an aversive conditioning pattern derived from meta-analysis to appetitive conditioning fMRI data. We tested pattern expression in three independent appetitive conditioning studies with 29, 76 and 38 participants each. During fMRI scanning, participants in each cohorts performed an appetitive conditioning task in which a CS+ was repeatedly rewarded with money and a CS- was never rewarded. The aversive pattern was highly similar to appetitive CS+ > CS- contrast maps across samples and variations of the appetitive conditioning paradigms. Moreover, the pattern distinguished the CS+ from the CS- with above-chance accuracy in every sample. These findings provide robust empirical evidence for an underlying neural system common to appetitive and aversive learning. We believe that this approach provides a way to empirically integrate the steadily growing body of fMRI findings across paradigms.

Brain responses to vestibular pain and its anticipation in women with Genito-Pelvic Pain/Penetration Disorder

OBJECTIVE: In DSM-5, pain-related fear during anticipation of vaginal penetration is a diagnostic criterion of Genito-Pelvic Pain/Penetration Disorder (GPPPD). We aimed to investigate subjective and brain responses during anticipatory fear and subsequent induction of vestibular pain in women with GPPPD. METHODS: Women with GPPPD (n = 18) and age-matched healthy controls (HC) (n = 15) underwent fMRI scanning during vestibular pain induction at individually titrated pain threshold after a cued anticipation period. (Pain-related) fear and anxiety traits were measured with questionnaires prior to scanning, and anticipatory fear and pain intensity were rated during scanning using visual analog scales. RESULTS: Women with GPPPD reported significantly higher levels of anticipatory fear and pain intensity. During anticipation and pain induction they had stronger and more extensive brain responses in regions involved in cognitive and affective aspects of pain perception, but the group difference did not reach significance for the anticipation condition. Pain-related fear and anxiety traits as well as anticipatory fear ratings were positively associated with pain ratings in GPPPD, but not in HC. Further, in HC, a negative association was found between anticipatory fear ratings and brain responses in regions involved in cognitive and affective aspects of pain perception, but not in women with GPPPD. CONCLUSIONS: Women with GPPPD are characterized by increased subjective and brain responses to vestibular pain and, to a lesser extent, its anticipation, with fear and anxiety associated with responses to pain, supporting the introduction of anticipatory fear as a criterion of GPPPD in DSM-5.
